Registered Nurse Case Manager
Place of Performance: Fort Bliss, TX
Specific Tasks:
- Ensure Service Members (SMs) are medically cleared for Mobilization/Demobilization (MOB/DEMOB) or ensure SMs are sent to the proper sections or units for their care. This is a very dynamic section requiring excellent time management, critical thinking, flexibility, communication, and teamwork. The use of SOP’s and published regulations are essential to succeeding in this environment.
- Seeing SMs is our top priority; wait time should be kept to a minimum. If the Case Management section is notified a second time that a Service Member is waiting, the Case Management leadership will assign that Service Member to an available Case Manager.
- Provide these services in support of all Service Members who are seen at the facility and who are identified as having a medical/surgical or mental health issue when processing through for mobilization/deployment or demobilization/re-deployment for any contingency operations.
- Shall see an average of 10 to 30 SM’s weekly and/or the weekly patient average of the other collective CPs, unless affected by circumstances beyond their control, which shall be reported to the Section OIC/NCOIC or designee.
- Shall comply with all MTF productivity standards and maintain a level of productivity comparable with that of other individuals performing similar services.
- Interview the service member, review the service member’s medical record.
- Unit trackers shall be updated daily.
- Shall assist in identifying service member needs and implementing the plan of care.
- Shall maintain documentation of all treatment provided in accordance with directives and prepare such records and reports as may be required by the policy. Documentation in MHS Genesis shall include, but is not limited to, all conversations whether in person or telephonic, guidance, recommendations you provide to SMs and their command teams. This includes any guidance, recommendations, and/or orders from the Provider(s).
- Shall prepare all documentation to meet or exceed established standards to include but not limited to timeliness, legibility, accuracy, content, and signature.
- Shall communicate pertinent service member data to Physician, Physician Assistant (PA), and/or Nurse Practitioner (NP) as appropriate.
- Shall carry out prescribed Physician, PA, and/or NP orders and document in service member medical records.
- Shall recognize and initiate appropriate measures in emergency situations, including initiating life-saving measures in absence of Physician, PA, and/or NP.
- Shall complete each encounter at the end of the day and appropriately close the record no later than 24 hours.
- Shall follow established government and professional clinical standards and accepted clinical protocols in the performance of duties as outlined in MTF policies and procedures.
- Shall abide by all clinical standards, rules, and procedures including requirements for licensure, credentialing, and quality assurance programs.
- Shall personally record hours worked in the Defense Medical Human Resources System – internet (DMHRSi) system, or other system, as required by the MTF.
Education:
- Associates or Baccalaureate Degree in
- Graduate from a college or university accredited by the Accreditation Commission for Nursing Education (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
Certification:
- Possess one of the following certifications:
- Commission for Case Manager Certification Certified Case Manager (CCM)
- American Nurses Credentialing Center Nurse Case Manager (RN-NCM)
- National Academy of Certified Care Managers: Care Manager Certified (CMC)
Experience:
- Possess a minimum of one year of experience within the last three years working as a full-time RN-Case Manager.
Licensure:
- Current, full, active, and unrestricted license to practice as a Registered Nurse in any State, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin
Additional Certification:
- Possess a current Basic Life Support (BLS) Certification. The American Red Cross or American Heart Association Basic Life Support for Healthcare Providers is the only program accepted. BLS recertification shall be maintained in a current status at all times.
