Pharmacist
Place of Performance: Fort Bliss, TX
Specific Tasks:
- Develop and participate in ambulatory care pharmacy practice sites and dispensing in Outpatient Pharmacy by developing collaborative partnerships with the medical staff and reviewing and monitoring outpatient medication regimens.
- Perform dispensing functions in accordance with established policies/directives.
- Assess patient’s response to drug therapy and planning drug therapy based on physician established
- Order and assess laboratory tests necessary to evaluate drug therapy effects and therapeutic
- Analyze patterns and trends to determine appropriate, effective, and economical drug therapy management and when improvement in these areas can be identified, recommends
- Modify dosages, or methods, or significantly alters regimen without prior approval of physician, but subject to post-action
- Provide clinical pharmacy services to selected clinics, including drug therapy management, patient education, therapeutic drug monitoring, staff education, and solicited drug therapy
- Document clinical pharmacy services and therapeutic consults which include economic impact on a continuous basis and prepares related
- Participate in quality assurance activities (e.g., drug utilization evaluations, measures of clinical pharmacist’s impact on drug therapy, ).
- Use of telecommunications and information technologies to provide health assessment, diagnosis, treatment, consultation, education, and health-related information across
- Complete each encounter at the end of the day and appropriately close the record.
- Shall provide pharmaceutical services for deployment and redeployment from theaters of operation.
- Shall provide pharmaceutical services in support of general military health readiness, along with support to the local MTF when coordinated between the MSU and Department of Pharmacy Leadership.
- Shall abide by all clinical standards, rules, and procedures including requirements for licensure, credentialing, and quality assurance programs.
- Shall comply with all health requirements, immunizations, and vaccinations.
- Shall complete all DoD, DHA, and MTF-specified training requirements.
- Shall personally record hours worked in the Defense Medical Human Resources System –internet (DMHRSi) system, or other system, as required by the MTF.
Education:
- Doctor of Pharmacy (Pharm.D.).
- Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
Internship/Residency:
- Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
Experience:
- Possess a minimum of one year post graduate experience as a clinical Pharmacist within the last three years.
Licensure:
- Current, full, active, and unrestricted license to practice as a pharmacist in any State, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
Registration:
- Possess a current Drug Enforcement Administration (DEA) Registration certificate and a National Provider Identifier (NPI).
Additional Certification:
- Possess a current Basic Life Support (BLS) Certification. The American Red Cross or American Heart Association Basic Life Support for Healthcare Providers is the only program accepted. BLS recertification shall be maintained in a current status at all times.
